The beeping of the heart monitor was all Twilight Sparkle heard in her dreams, along with it ending and the pain coming again.

If only her friends were there, she would be happy. But they've gotten a little better at this, and now it is time for them to come back to her. But Twilight knows she is immortal, and she would never be up there with them. Sometimes our decisions affect everyone we know. . .
