The orange mare wakes up to find her self lying on a metal table of some sort. She tries to move but is restrained by rough leather bindings. Then she sees the rack of medical, and not-so-medical devices. She sees Twilight over on an identical table with rack. Then HE walks in from an unseen door.

"Hello my name is Daniel, I'll be your doctor for today", was all he said before IT began.

He walked over and grabbed something from the rack, it was a tooth puller. He silently held her mouth open and reached in and grabbed her tongue with the dental pliers. It was slow, at first anyway, then he started playing with it, jerking it roughly from side to side with the dental pliers. Then he pulled, tearing the muscle out of her mouth, and quickly shoving it down her throat with his hand, forcefully dislocating her jaw in the process. Then he quietly put the dental equipment back in it's place, pulling out, to the mare's dismay, a jar of razor blades. He was wearing heavily padded gloves, so he just shoved his hand in and grabbed a handful. In his other hand he held a scalpel, with which he splayed open her stomach and shoved in the razor blades. Causing excruciating pain after wichhe injected her with fluids to keep her awake.

She laid there on that table, slowly bleeding out, when suddenly Twilight woke up and the man was moving towards her and someone burst in... but that's when she blacked out from the loss of blood.
