A single tear rolls down your cheek and your hooves start to shake as you continue to stare at the test, hoping that it miraculously changes to "positive" - but to no avail. For several years you tried to get pregnant, waiting for your estrus and making preparations, finding a mate beforehoof and receiving several "fillings" of your mate's seed and keeping it in you as long as possible. But it doesn't seem to work.
What am I doing wrong?
You stare out of the window, the sun illuminating the world outside of your house. You see children playing outside, their parents chatting with each other, smiling. Another tear rolls down your cheek, bigger than the first. You turn your head and look in the mirror. All you see is your saddened expression. You massage your belly and think to yourself.
I always thought that it's the stallion's fault, that... that all of them have been... impotent or incompatible with me. But... I still have no... child.

A week has passed since you first visited the hospital to get a checkup. You sit in an examination room which is relatively small and packed with apparatuses you can only imagine their purposes. The door creeks open and Nurse Redheart enters the room. She looks at her clipboard that she holds in her magic aura and closes the door. She looks up to you and smiles.
"Hello dear, how are you feeling?" she asks in her caring voice.
You look down and caress your belly, then back up to her. "Empty."
The nurse's smile fades. "Oh dear... I wish I could say that I understand."
She closes up to you and embraces you in a tight hug. All you can do is sniffle as a tear rolls down your cheek. Not because you feel empty, but because somepony cares about you. You don't mind that she cares because she's a professional, the hug feels too good to mind. Too good to hold back your feelings. She breaks the hug and looks at you in a warm manner.
"I have good news for you. The blood tests didn't return any results that are alarming. But..." once again the warm smile fades and a look of worry takes place in Redheart's face. "We still don't know what is wrong."
She hands you the clipboard which you grab with your hooves. Your eyes wander over the document, but all you can understand is your name and your birthday, since everything else are medical terms and values you don't understand. You give the clipboard back to Nurse Redheart who places it on a counter.
"I'm going to perform some diagnostic spells on you, are you okay with that?"
You simply nod in return.
"This might tingle a bit. if it gets too intense just tell me and I will stop."
She lowers her head and points her horn to your abdomen. Her horn starts to glow as a small beam finds its way to your belly. You gasp at the sudden tingling intrusion. First it's just your skin that is hit by the beam that tingles, then it gets deeper, tingling its way into your core. You start to shiver as the tingling experience gets to your ovaries - at least you think that it reached your ovaries. You can feel the beam exploring your inner depths and soon you find that your whole nether organs are tingling. Suddenly the feeling changes to a feeling that could be best described as massaging. The nurse looks away, a worried expression decorating her face. "Well... you shouldn't." You blink several times, trying to comprehend what she just said. Before you can say something she continues, looking back at you.
"What you experience is a false estrus. You just do what other mares do without you knowing about it. I hate to break it to you, but..." She closes her eyes and lowers her head.
"You are infertile."
Your mind races.
Infertile.
That word. Your world collapses around you. You realize you'll never get pregnant. You'll never be able to hold your foal, your own foal. You'll never see it grow, see it say it's first words and steps...
This realization hits you. Your legs give out and you fall from your chair onto the floor, a blank expression on your now tear streaked face. You just lie there, just breathing and crying as you feel a hoof on your side.
"I'm... I'm sorry... I can only imagine how hard that must be... Your ovaries are just... not working, I don't know why... but maybe there's something you may be happy with."
The nurse flinches slightly. She hoped you take one of the children from the orphanage. Estrus leaves a lot of unwanted children.
"There's still another way." You look up to her, still frowning.
"You would help foals who don't have a mother by nursing them."
You open your maw to protest, but Redheart cuts you off.
"Listen before you reject this offer." Since angering the nurse is a bad idea you simply nod.
"You'd nurse the foals, either by milking via a machine or by directly nursing them, whatever you are comfortable with. The hospital pays you for your work and you get to help children in need." She caresses your mane to comfort you and silly as it seems - it works. Your breath slows down to a normal rate and you stop crying.
You look into her eyes, the pleading look nearly forcing you to agree. "What's the catch?"
She starts to smile warmly again as she realizes that you don't deny directly. "The only downside is that your teats will swell - obviously. But we have trained masseuses who will help you with that."
Nurse Redheart stands up and opens a cabinet, holding a small tube in her magical aura. "If you should agree to this, this will help you." She sets the tube down into your hooves. "This creme will soften the skin of your teats, making the stretching less painful." The nurse walks to the door and opens it, looking back at you. "Apply this for a week, twice a day, and come by again then. We will discuss everything else after that, okay dear?"
You look at the tube and then at the nurse. Nodding with a smile you respond "Will do."
Maybe it's not as bad as I think. Nursing foals is probably the nearest motherly feeling I'll get, so why not. I'll give it a try.

A New Hope

The doctor turns her attention back to the topic at hoof. "I'm Doctor Ray and I'm happy you decided to take this course of action." She lifts her right forehoof and rests it against your withers. "I truly am happy that you'll help foals in need, but until then it's a long way. Of course you'll get your payment from today on, if that's important to you."
You shake your head. "No, I just want to have motherly feelings. Money is just a nice side effect to me."
The doctor smiles again. "You will have them in due time. Do you have any questions before we start?"
You rub your chin with your forehoof. "Hmm... Ah! Yes. Do I have to... get milked in the hospital? Or... how does the whole thing work?"
Doctor Ray nods and answers. "For the first week we want you to get to the hospital every day, to see how your teats are developing and if everything develops as planned. You'll also get a milking machine today for your home. After that we expect a visit once a week, again for checkups. A delivery pony will collect your milk and deliver that to us, where we will examine it. We have to know if the milk contains any unwanted bacteria or diseases."
You look shocked. "But you tested my blood before? Why do you~"
She cuts you of. "We know, and right now you are fit as can be. But maybe you'll catch a flu? You'll never know." You nod as you understand. "When the milk is clean we give it to the orphanage. You'll also receive a bonus for the amount of milk you give and your monthly payment will be raised if you get other mares to join our program - we really need milk."
Once again you open your snout. "But... why not take milk from cows?"
"Well, you see, the milk given by cows is meant for calves ad therefore contains specific nutrition and fat that is needed for calve that need to grow and gain fat. Foals however don't need that much fat. Also milk given by ponies contains higher background magic for the development of the magic abilities of earth-, pegasus- and unicornponies alike."
"Ah, that makes sense." You say as you look to the side. From the corner of your eyes you see the stallion anatomy poster and decide to look at her and think of something unpleasant. "So, what's next?"
There's a sparkle in the doctor's eyes. "First I'll give you an introduction on the milking machine and then I'll cast a lactation inducing spell on you." You nod softly. "All right! This..." She walk to a small table and points her forehoof to a machine. "is out standard milking device. It has two suction cups that are connected to a small jug where the milk will be stored." She glances to you. You nod and she continues. "This gem powers the device, creating the vacuum that is needed to milk you." You have no idea about magic gems, so you just nod - again. "The application is pretty easy. You sit down on your haunches, hold the cups against your teats and push that button." She pushes said button down and the machine starts. It whirrs softly but suddenly stops after a few moments.
"Why did it stop?"
"The machine detected no teats to milk."
Wow... that machine is smarter than some farmers. "And how do I stop it?"
"You don't need to. When you have no more milk to give it will stop. But if you need to stop it, just push the button again. It's that simple." She looks back at you. "Anymore questions?"
You stare at the machine in awe. "Uhh..." you turn to the doctor and close your snout, just to open it again to speak. "No, not yet."
Doctor Ray smiles again and clops her hooves together in excitement. "Great! So, let's get started then, okay? Please lie down on the bench." You turn to the bench and hop on it. "On my back?"
"Yes, please." The bench is wide enough, so you lie down on your stomach and turn around, your underside fully exposed. Your faces flushes in a bright red.

He takes a few steps and then stops to look at you again. "Uhm... where is your living room?"
"Follow me." You walk towards the living room, Cubic following behind you. Cubic, however, seems to enjoy both the smell and the looks, walking closer to you to get more detail of the beautiful landscape in front of him. Both of you arrive in the living room, a room with a nice sky blue carpet and wooden walls. It contains two couches and a coffee table and various picture frames, showing your relatives and friends.
"Please lie down on the couch." The grey stallion says, his hoof pointing at one of the two couches. You hop on it and lie down on your stomach. Cubic tilts his head and frowns. "You need to lie on your back."
You look at him and frown yourself. "Why?"
"Because... well... your teats are on your underside." he scratches his neck with his right forehoof. This situation is clearly awkward for him. You decide to switch into a sitting position: Your rump on the couch cushion and your back against the backrest of the couch. Your tail hangs down and flicks nervously, trying to hide your assets that are in plain sight for Cubic to stare at. You clear your throat, causing Cubic to look up into your eyes, blushing furiously. "Uhmm... yes... uh... milking..." He turns around and lifts the machine on the couch, detaching the cups from the machine.
"First we need to hold the cups against your teats, and then we~" You interrupt him with a gasp and a kick against his snout out of reflex, as the cold cup brushes against your soft flesh. Both of you lift their forehooves to your and his snout: you out of surprise and Cubic because his snout hurts like Tartarus.
"Ah! FUCK! And Doc told me to preheat the cups with my breath! Ugh.... should have seen that coming."
"I'm sorry, you startled me with those cups and~"
He cuts you off with a wave of his hoof. "It's okay... it's my fault." He looks at his hooves to check if his nose bleeds, but sees that it doesn't. With a sigh he picks up the cups again and breathes softly against them, warming them. You observe the unicorn stallion, his horn glowing slightly to hold the cups. As he finishes with both cups he looks up to you, the cups hovering above your teats.
"Are you ready?"
