After a freak accident leaves him with severe brain damage a stallion named Smalls is sent to a mental health institute on an island off the west coast of Equestria. His condition is grave, its something he may never recover from, but that won't stop him from trying to communicate with the one person who listens, a mad pony in the lower ward.
