Apocalypse survivor Javier Garcia receives an unfortunate head injury, and suddenly, whenever he falls asleep or otherwise loses consciousness he's no longer there. Whether he's knocked out in the middle of a fight with other humans or heading to bed for the night, he wakes up in a strange land filled with colorful talking horses.

A land where there's no dead walking.

Then it happens in reverse. Go to sleep there, wake up back home. Over and over again.

He begins to question his sanity, finding items from the magical world appearing with him in the world where the dead walk, and vice versa. He hasn't tried transporting a person, and isn't about to; The land is a paradise, but not for the likes of them, for every human is infected. Whether Equestria is a real world or a fantasy brought on by his head injury, Javier isn't going to let this world suffer for his own selfish wants.

Only time will tell if it works.

This story's pretty much being written by the seat of my pants. Things will get gruesome later on but I'm going to try and take it slow.
