At the Hospital in Manhattan, the moment they brought Twilight in, the staff brought in a stretcher before taking the purple alicorn to the emergency room. As for Pinkie Pie, they took her to the one of the rooms at the 4th floor inside the hospital. As for Applejack, they performed a bit of medical treatment for her injured eye and skull. After that, the farm pony received an eye patch so that she'll recover from her eye injury in a few days. For Twilight, they performed several counts of brain surgery and wrapped a bandage around her back. After being treated, Twilight was sent to another room at the same floor where Pinkie is to rest after this night's Purge. After several weeks of recovery, Pinkie and Twilight we're both able to leave the hospital and met up with the rest of their friends at the train station before heading back home. Unfortunately, because of their terrifying experience from the 3rd year's Purge, Pinkie Pie didn't hone any victory parties until one month later after being traumatized when facing against one of the purgers. At the same time, Twilight had episodes of brain damage, memory loss, and lack of speech that lasted until her head injuries finally disappeared.

Upon hearing all this, Pinkie's family and Twilight's parents couldn't help but worry about the situation both of their daughters are in. In fact, Shinning Armor was feeling very concerned about both his wife and Twilight Sparkle for letting the criminals in Manehattan take advantage of the Annual Purge. But what's even more disturbing is that Shinning could hear his wife cackling in a evil tone from an isolated room inside the Palace back at the Crystal Empire. At first, Shinning didn't want to think that there is something wrong with Princess Cadence. However, after those two years, Shinning is starting to suspect that his wife is up to something that may put their country in danger if they keep continuing with the Purge. So during that time, Shinning ordered his servants to take his daughter Flurry Heart to a daycare center in Ponyville to insure her safety. For Night Light and Twilight Velvet, they were hearing a lot of angry protesting that is aiming towards their daughter every week. They could not help but feel sorry for Twilight Sparkle for being forced to commit such bad decisions against their own will. Despite all this, the Sparkle Family can only pray that Twilight will do the right thing for their beloved country someday.

As promised, Twilight and Rarity are both able to collect the bounty for Yeo's capture and this time, they relocated Rarity's Manehattan boutique several miles west outside of the city. Shortly after Yeo was detained at the Manehattan Police Station, they sent him to another prison area in Saddle Arabia to serve his life sentence. For Suri Polomare, she took off to the South of Manehattan to start a new fashion career to parts unknown and was never seen again. Rumor has it that Suri proceeded to participate in other fashion designer contests with the other fashionistas apart from Rarity so that she doesn't feel outsmarted again. After they got Rara, Cheese, and Coco out of Manehattan, they were able to seek vacancy in Twilight's Castle back at Ponyville. For Babs Seed, after she was reunited with her Orange relatives, they decided to leave their country house and move to Sweet Apple Acres and live in another house that is next to where Applejack's family lives in order to avoid putting their family in danger when it comes to the purge commencement.

After going over the death count in Manehattan, Twilight discovered that 250 ponies have been killed during the 3rd annual purge. Outraged by such horror, may ponies were making angry protesting against Twilight and her fellow princesses all over Equestria like a huge tidal wave of hated. Desperate to make it up for all the ponies that were suffering under the hands of the Purge and the NFFE, Twilight immediately requested help from her brother, Shinning Armor, her personal bodyguard, Flash Sentry, and from her student, Starlight Glimmer, to find another political group that can help her abolish the purge before it starts again and to deal with the overpopulation in a more civilized and nonviolent way. Sadly, after 9 months of searching for suitable candidates, no pony was brave enough to help abolish the purge for they feared that the NFFE would come after them by sending their assassins to do them in. For the rest of the Mane 6, they had nightmares of them getting slaughtered by the purgers for almost every month. For Fluttershy, she suffered the worst nightmares out of the Mane 6 claiming that she has seen her animal friends getting butchered up for food for the griffins and Minotaurs before they stabbed the yellow pegasus from behind, while Pinkie Pie gets stalked by a serial killer during each party that she hons for her friends and for her fellow ponies. Upon learning that her friends' nightmares are getting worse every month, Twilight is starting to lose hope of finding a suitable candidate to help her end the purge in Equestria.

One day later, somewhere on the 10th month of the 4th year, as the sun sets from the sky, Twilight was about to take a rest from her stressful day of Royal Duties in Ponyville. When she enters the Main Foyer inside her Castle, Twilight meets up with Applejack who is sitting on a bench to her right. As she looked closer to Applejack, Twilight notices that the farm pony isn't wearing her eye patch anymore. However, Applejack's injured eye has changed color from green to red in which is the result of recovering from her facial bruises after the 3rd Year's purge. For Twilight, even though she is no longer wearing a dress wound covering up her head or a patch covering up her cheek , she still has a square patch covering up her back. So, every time she moves around while wearing that patch, Twilight could feel a small burning sensation coming from her back which makes a small painful swelling that lasts for only a few minutes; during that time, Twilight starts making small panting and grunting coming out of her lips. Before Twilight could say anything to Applejack, the farm pony makes her speech first.

"Hey Twi," Applejack asked as she see the purple alicorn sit beside her. "Are ya feeling okay??"

"I don't think I am Applejack!!" Twilight answered in a depressing tone. "It's been 10 months since I ordered my friends and subjects to find a suitable candidate to help me abolish the purge. I don't know if I can continue this any longer even after three years. I mean, this enactment is affecting us Element Bearers of Harmony all at once. I mean, we almost got killed out there during that night and......"

"It doesn't matter Twi," Applejack replied while trying to talk some sense into her purple alicorn friend. "You already told us that ya promised yerself ta find some pony else to help ya abolish this err purge as yer one and only chancellor, right?!"

"Yes, but it may be easier said than done!!!" Twilight explained to the farm pony mare. "If we don't find some pony to run for chancellor in order to do this, more ponies will die because of me. I just everypony down and this maybe the end of my friendship as we all know it."

Before Applejack knew it, Twilight started breaking down in tears as she covered up her face with her front hooves. After seeing Twilight cry for several minutes the farm pony embraced the alicorn in her hooves.

"There. There. It's alright sugarcube. We'll git through this together, even if dis is the last thing dat we ever do!!!" Applejack said as she held onto Twilight while still sobbing.

After embracing each other for a few hours, Applejack slowly let go of Twilight before she headed out the doors.

"Look, we'll work something out tomorrow hon okay?" Applejack suggested to Twilight before leaving her. "For now, just git some rest, ya. We still have a big day tomorrow."

After seeing Applejack disappear out the doors of her castle, Twilight walks up the stairs and into her bedroom with tears coming down from her eyes. During her trek through her castle hallway, Twilight felt depressingly traumatized after all of that killing that she and her friends caused during the 3rd year's purge. As a result, their actions during the purge almost costed them entirely, despite how they saved most of their loved ones from getting killed by the purgers. Not wanting to think about the negativity revolving around the purge, Twilight gets on top of her bed before she slowly closes her eyes and goes to sleep.

In the morning, as soon as Twilight wakes up from her beauty sleep, she gets up from her bed only to spot a letter on top of her dresser across from her bed. As soon as she approached the dresser, Twilight used her magic to levitate the letter off from the top of the furniture before reading the its contents. When she read the letter from top to bottom, Twilight eyes widened with shock and surprise. After reading the letter, Twilight has just discovered a clue that explains a dark conspiracy that is unfolding before her very eyes, which was done by both the NFFE and Queen Chrysalis, her old nemesis that almost took over Canterlot during her brother's wedding.

"Those dirty bastards!!! I should never have trusted them!!!" Twilight snarled to herself as she packed up her bags before heading out of her bedroom. "I need to show this to Shinning Armor immediately."

As Twilight dashed out of her bedroom, along with the letter, she went down the stairs and head out to the Castle's main entrance. In an instant, Twilight went out the castle doors, unfurled her wings, took off to the skies, and flew to the Crystal Empire. Unbeknownst to her, two ponies appeared from the shadowy corners of Twilight's Castle foyer while wearing brown cloaks. The moment they looked at Twilight before the purple alicorn disappeared into the sky, the two cloaked ponies stared at each other before they could say anything.

"So Charlie, are you sure she can be trusted after she helped the NFFE enacted the purge??" The male cloaked pony asked the mare on his left.

"Yes Leo, because I knew that Princess Twilight needed help to restore her friendship and harmony," The cloaked mare answered while wearing her hood. "It's also because I planned to help Princess Twilight save the real princesses from their captivity first before we conduct our operation to go after the NFFE and stop the purge in Manehattan!!"

"Well, we might as pray that she'll be able to make it up to all the ponies that she did wrong, as well as dealing with those false princesses!!!" The cloaked stallion acknowledged.

After they're done with their conversation, the two cloaked ponies took off to the river outside of Ponyville and went on their getaway boat before heading out into the daylight.

During her flight, Twilight could not help that her fellow princesses are in danger not from just the purge but also from getting captured and forced to watch ponies hurt and kill each other unwillingly. Also, Twilight couldn't stop thinking about the words that she read on the letter:

"Dear Princess Twilight Sparkle,

We heard that you're trying to get rid of something that you were forced to carry out. But there is something far worse going on here than just the purge occurring at Manehattan. Also, your fellow princesses aren't what they seem two years ago. Meet us at the Manehattan City Hall and we'll explain everything there revolving around Queen Chrysalis, the NFFE, and Caleb Warrens, as well as their dark conspiracy against the Equestrian crown. But don't tell any pony else other than your brother about it."
