Room 3-24, Ponyville Hospital
Entering the operating room, Twilight, Spike and Thorax were lead inside by the earth pony doctor when they spotted 3 other ponies standing by a medical bed, Twilight and Spike were familiar with one of them from a previous encounter, calling out to him. "Doctor Horse, please tell us everything that happened. We-"
"Where's Ocellus? Is she going to be all right?!" Thorax cried as his wings buzzed in panic.
Doctor Horse glanced towards Thorax as he replied somberly. "Sir, we ask that you remain calm-"
"Remain calm? Remain Calm?!" Thorax growled bitterly. "How can I be when one of my own hive is likely in serious pain?"
"Thorax, stop!" Spike spoke in response, "He's just trying to do his job, don't take it out on him."
"But Spike, O-Ocellus she-"
"Thorax," Twilight interjected. "I-I'm sure she's fine... just..." Turning her head towards the unicorn doctor she continued, "She is fine ...right Doctor?"
Doctor Horse sighed, "Physically yes, she's as fit as a fiddle since the day she was born err..." He looked towards the male changeling. "Hatched?" Thorax nodded. "Yes, since that day she's been physically fit for a changeling of her caliber."
Twilight spoke up, "Well, what about mentally? Do you have any insight on her mental state?"
"I err... that's the reason I brought you all in here a-actually. I-I um..."
One of the ponies standing by the hospital bed, a unicorn mare replied. "What she means to say is..." Turning towards the unconscious changeling, she sighed nasally. "It's best if you come over here so you can see for yourself."
As Twilight and co. were walking over towards the medical bed, Thorax spotted two wooden monitors laying out two different readings. One was a simple heartbeat monitor that showed her heart was working as fine as ever, but looking over towards the second monitor relayed two different readings with both of them completely flatlined.
Noticing this, Thorax spoke in concern as he pointed towards the respective screen. "D-Doctor? W-what is that monitor for?"
Twilight and the doctor turned towards where he'd pointed as she had a grief-stricken look, "Oh no..." Turning towards Thorax, she slowly walked over to him teary-eyed before reaching up to his neck pulling him into a hug. "I'm sorry... I'm so... so sorry..."
The Interdimensional Plane
As Oscar's soul flew through the endless void, he stumbled upon a colossal glowing entity as they spotted him. Oh hello.~ What do we have here? Another soul wandering the dimensional plane?
The man groaned from his recovering alcohol induced headache as he looked up backing away in alarm, "Whuh I- uh um uhh..."
The colossal titan chuckled at his reaction. You need not be afraid, I wish no ill intent upon you. Using her tendril arms, she brings him up to her field of vision. I am after all the guardian of this realm. Surely you've heard of me from the tales of olde, have you not?
Oscar shook his head, "I think I would've heard stories about a gigantic purple god thing had they'd been written. Erm... sorry."
The lavender behemoth sighed. It seems not everycreature is aware of my existence after all... but that doesn't mean I can't change yours. Using one of her tendril arms, she rips open a small pocket into a nearby dimension as Oscar spots a hospital off into view. Go now, I think you deserve a chance to live once more.
"A hospital I- what do you? Nonononono wait!" And with those final words, Oscar's soul was flicked off into the nearby plane as the Holy Titan waved him off.
Room 3-24, Ponyville Hospital
As the alicorn wept on Thorax's shoulders, Spike replied timidly. "W-w-wait, d-does that mean she's..."
"No... NO! She can't be... her... I-" choking back a sob, the changeling king bolted out of the room as Spike called out to him. "Thorax‚ wait!"
Feeling something feathery prod his shoulder, Spike looked back to see one of the doctors looking at him. "It's best to let him grieve in peace alone, don't let it bother you too much." He sighed. "These things happen, sad as it may be..."
Everypony turned to face the Doctor Horse as he drew their attention. "Ahem. Regardless, we still have a job to do." Turning his head to face Twilight, he continued. "Would you two please step outside for just a moment? We need to finalize some private documents if you don't mind."
Twilight nodded, "Of course, come on Spike. Let's get going."

After Twilight and Spike exited the room, the doctors were busy finishing up the remaining documentation as the unicorn mare sighed. "I don't know how you guys keep it up with how tough it is to tell them the bad news, your friend/family member is deceased? I just couldn't imagine anypony outright saying that."
The earth pony replied, "W-well I-it's not like anypony told them that specifically, we just..."
"We told them without actually telling them," The pegasus stallion turned to face her. "Isn't that right, Tenderheart?"
"Dr. Caduceus!" The unicorn mare shouted, "Please be a bit more sensitive around her about this, we don't get many incidents like this in Ponyville you know."
"Alright, Alright. Lay off me would ya? I was just being honest..."
While the two of them were busy squabbling, a beam of light from an open window flew into Ocellus as the light quickly disbursed.
"Alright, alright, enough you two." Doctor Horse sighed, "Let's get back to work."
