The history begins with Detective Twilight Sparkle and her co-workers, Starlight Glimmer and Sunset Shimmer, are summoned to a recent crime scene at Crystal Psychiatric Hospital, while returning from solving a case.

Along the way, their car radio emits a mysterious high-pitched sound as the team nears their destination.

Arriving at the scene, the trio discover that a bloody massacre has taken place inside the asylum, and the place seems deserted.

Leaving Sunset behind, along with their driver Vinyl Scratch, Twilight and Starlight continue to the mental hospital's security room, where they find a woman named Abacus Cinch, traumatized but alive, who is apparently the only survivor.

As Starlight tends to Cinch, Twilight checks the security cameras, where she witnesses supernatural murders at the hands of a mysterious disfigured and hooded woman, who then appears behind Twilight and attacks her, knocking her unconscious.

Later, Twilight wakes up in the basement of the asylum, kidnapped by a mysterious masked man armed with a chainsaw.

As she manages to steal a key and escape towards the exit, Twilight trips over a wire, which sets off an alarm that alerts the killer of her escape attempt, who then paralyzes her with a chainsaw wound to her leg.

Though badly injured, Twilight manages to fight her way through the many traps set in the basement and a deserted treatment room, where she sneaks out and makes it to the reception desk, though the chainsaw killer notices at the last moment, and he chases her to the elevator.

Despite her leg injury, Twilight manages to successfully escape in the elevator, as the chainsaw killer roars in rage.

Back on the surface and leaving the hospital, Twilight reunites with her teammates, who had managed to find another survivor named Fluttershy.

When Twilight notices Starlight's absence, Canterlot City begins to deteriorate.

Then, despite the best efforts of escaping by ambulance, the hooded woman suddenly reappears inside the ambulance as another high-pitched sound is emitted, transforming Vinyl into a murderous creature and causing the ambulance to skid off the road and crash in a forest.
