When Twilight manages to escape from the underground catacombs under the church, she arrives at an ancient cave system that is teeming with monsters.

As she moves forward, she is suddenly overwhelmed by a strange wave of energy that teleports her back to Crystal Mental Hospital, where she is reunited with Abacus Cinch, who is working on a similar machine that Pinkie used on Fluttershy in her vision. .

There, Cinch reveals that they are all trapped in a world created inside Pinkie's dead mind, which is the epicenter of a machine called STEM, and that Pinkie plans to kill them all.

At that moment, and before Cinch reveals more, the machine explodes again, knocking Twilight unconscious and separating her from Cinch.

When she wakes up, Twilight hears a deafening roar behind her and is quickly chased by a new, large, misshapen monster.

Twilight quickly manages to outrun the creature and return to the mysterious office where Spike works... only to be knocked out again by an unexpected appearance by Pinkie.
