Following their battle with the chainsaw killer, Twilight and Cinch arrive at a hospice yard owned by Cinch's brother, Sombra, as they continue to search for Fluttershy.

As they proceed, Cinch reveals that Fluttershy was a patient under Sombra's care, until she was transferred to Crystal Psychiatric Hospital for further treatment.

Searching the main building of the hospice, the duo are attacked by Sombra, who had turned into a monster, and Twilight is forced to kill him.

The pair then find Fluttershy hiding in the basement, only for the trio to be ambushed again by the mysterious hooded woman, whom Cinch recognizes as someone named... Pinkie Pie.

As she approaches Pinkie, Twilight is teleported into a hallway, separated from Cinch and Flutteshy, and then ends up in a large underground dump after being swept away by a tide of blood.

Afterwards, Pinkie reappears and attacks her with a group of creatures to kill her, but Twilight manages to emerge victorious against them, though she is then immediately stalked by a mysterious multi-armed creature.

Believing to kill the creature with fire, Twilight continues onward, only for Pinkie to reappear and cause the walkway Twilight is standing on to collapse and fall into the abyss.

After going deeper into the town, Twilight finds Fluttershy and learns the name of the mysterious hooded woman, Pinkie Pie.
