After waking up from the long fall, Twilight discovers that she is in what appears to be an abandoned hospital, though she soon realizes that she is not alone: there are creatures in the corridors that can turn invisible.

After defeating them, Twilight is forced to solve a deadly puzzle, created by Pinkie.

After solving it, a loud blood-curdling scream suddenly erupts from a nearby wall, where bloody handprints constantly knock until revealing a door.

Walking through the door, Twilight enters a small room with various medical equipment, along with what appears to be a bathtub, where she finds an unconscious Starlight Glimmer.

Worried about Starlight, Twilight wakes up her friend and they both move on, while Starlight reveals that she doesn't remember since Pinkie attacked Twilight in the asylum.

Just as she says this, another high-pitched sound stuns both detectives and some kind of mutation manifests in Starlight, who promptly attacks Twilight.

Though the effects wear off quickly, Starlight is left terrified by the experience of almost killing her friend.

As they make their way through the hospital hallways, Twilight and Starlight make it to a large room, where they hear a familiar voice screaming for help: it was Sunset Shimmer, who was trapped in a Plexiglas tank that was slowly filling with water in order to drown her.

As Starlight runs to rescue Sunset, hordes of the creatures appear and attack the group, though they are defeated.

But, when Starlight analyzes the machine, she discovers that there is no way to free Sunset from the tank, forcing Twilight to manipulate a control panel from above with Starlight's help.

As soon as the panel is tampered with and the tank is opened, both Sunset and Starlight fall into a newly opened hole in the ground, forcing Twilight to jump into it to search for her companions.

After briefly reuniting with Sunset and Starlight, Twilight is once again forced apart from them when an unseen entity grabs her and drags her down a creature-infested tunnel.

Then, as they head toward the end of the tunnel, the monster of the arms reappears, forcing Twilight to fight again.

Finally, Twilight wins and leaves the tunnel, only to find herself in what appears to be an old laboratory, where she sees a vision of Pinkie pondering Fluttershy's viability as a test subject for her "experiments".

When the vision ends, Pinkie resurrects the corpses connected to the lab's machine for they kill Twilight.

Quickly, Twilight manages to disconnect them from the machine, killing them, but an explosion in the machine renders her unconscious.

Author's Note

In what appears to be an abandoned hospital, Twilight is temporarily reunited with Sunset and Starlight, as she finds out more about Fluttershy and Pinkie Pie's relationship.
